  • Position Overview
    • Manages the nursing needs of assigned patients from admission to discharge ensuring patients receive quality nursing care provided in a financially responsible manner. Responsible for the provision of skilled nursing services and teaching in the home. Works with the Nursing Coordinator and the interdisciplinary team to develop and implement a patient specific plan of care in accordance with the physician’s orders.
  • Position Requirements
    • Education/Experience
      • Minimum Required:
        • Graduate of an approved RN program OR a non-approved program supplemented by content equal to an approved program.
        • Two years experience as a Registered Professional Nurse required OR equivalent of education and experience.
      • Preferred:
        • Bachelor degree in Nursing.
        • Med/Surg/Certified home care experience.

    • License/Certification
      • Minimum Required:
        • License and current registration to practice as a registered professional nurse in New York State.
      • Preferred:
        • Not Applicable.
