Position Overview

  • Responsible for all clinical/patient scheduling to assure efficient time management for staff and patient care.

Position Requirements

  • Education/Experience
    • Minimum Required:
      • High school diploma or equivalent.
      • Computer Skills.
    • Preferred:
      • Experience in Scheduling.
      • Home Care Experience

Work Environment

  • Most work is performed in offices that require working in close quarters with other employees. The offices are air conditioned/heated.

Position Functions

  • Essential Functions:
    Essential functions are those tasks, duties and responsibilities that are performed with or without reasonable accommodation.
    • Schedules visits for Skilled Nursing and Home Health Aides by plotting visits and calling patients.
    • Schedules admissions by coordinating with Nurse Manager to assign clinicians to patients.
    • Triages by reviewing unscheduled visits and then assigning to appropriate clinician.
    • Communicates as needed with clinicians via e-mail, voice-mail, Blackberry and Ndoc; Follows up on physician orders as needed.
    • Participates in appropriate agency meetings.
    • Supports the Corporate Compliance Program by adhering to the Twin Tier Home Health and Professional Home Care Policies, Procedures & Department Manuals of the organization. Employee will attend all mandatory in-services and annual training programs relating to Corporate Compliance. Assures that activities that employee is responsible for compliance with any federal and state regulations. Reports and corrects any such conflicts which may arise.
    • Demonstrates knowledge of and adheres to Twin Tier Home Health and Professional Home Care policies and HIPAA regulations governing confidentiality and security of protected health information for both manual and electronic patient records, charts, information, and other confidential information including (but not limited to) employee records, computer access codes and passwords.
