  • Position Overview
    • This position provides psychosocial support for the needs of residents based on a comprehensive assessment. Assures residents the maximum attainable quality of life and emotional/physical well-being in compliance with all Federal and State mandates. Completes resource referrals as needed, and case management services to residents and families; works in collaboration with the multidisciplinary treatment team to incorporate psychosocial needs and concerns in planning care; assists in developing resources to meet patient and family needs; participates in resident, family, staff orientation, education and in-service activities to promote understanding of psychosocial issues. Accountable for intervening with residents and families, by applying critical thinking skills to monitor psychosocial status and issues that impact the clinical progression and transition/discharge plan for residents, and support of all long-term care residents, including the facilitation of patient and family communication, coping and decision making. Works as a part of a multidisciplinary team
  • Position Requirements
    • Education/Experience
      • Minimum Required:
        • Bachelor's Degree in Social Work with 1 year experience in a healthcare setting
      • Preferred:
        • Master's Degree in Social Work (MSW)
        • Minimum of one year of experience working with geriatric population.
        • Knowledge of clinical social work skills, values and philosophy.
        • Working knowledge of long-term care regulations, MDS 3.0, Medicare and Medicaid.
        • Working knowledge or experience in the Clinical Supervision or Support of BSW
