Social Worker, Chenango Bridge

United Health Services is seeking a Social Worker to join our Pediatrics team in Chenango Bridge, NY. This is a [BENEFITS-ELIGIBLE position.

Social Worker Overview

The Social Worker incorporates social work functions and skills into the primary care setting. Provides counseling or care coordination efforts are a key function of this position while additionally providing liaison services to CPS/DSS.

Social Worker Key Responsibilities and Expectations

  • Collaborates with Clinical team
  • Identifies specific issues / special needs for individual
  • Conducts case management assessments
  • Contributes to formulation of treatment plan
  • Discusses discharge planning throughout treatment
  • Participates in formulation of discharge plan as identified in outpatient standards
  • Participates in case conferences
  • Provides consultation services
  • Demonstrate commitment to organizational Values of Compassion, Trust, Respect, Teamwork, and Innovation

Position Qualifications

Minimum:

  • NYS Licensed Master Social Worker (LMSW)

or

  • NYS Licensed Master Clinical Social Worker

Preferred:

  • Pediatric Psychology Experience
  • LCSW-R credential
