Home Care Registered Nurse (RN), Binghamton Area

United Health Services is seeking a Home Care RN to join our Twin Tier Home Health Team in Binghamton, NY. This is an in home, traveling nurse position; each Registered Nurse will travel throughout the 4 counties in the Lower Tier area of NY State This is a BENEFITS-ELIGIBLE/36 Hour/week Salary position. This position offers much flex in schedule. This position offers flexibility in schedule. Great for RN's with busy lives and families!

- $43.86 hour, depending on experience, plus mileage reimbursement.

Home Care Registered Nurse (RN), Overview

The Home Care RN provides skilled nursing services and patient education in the home care setting. The Home Care RN makes the initial assessment visit in the home to determine whether the patient is appropriate for admission to the Agency. Develops and implements the plan of care in accordance with physician orders. RN’s will be expected to travel (average expected travel 25-50 miles/day)

Home Care Registered Nurse (RN), Key Responsibilities and Expectations

  • Makes the initial assessment visit in the patient’s home and determines whether the patient is appropriate for admission to the agency. Develops tentative plan(s) of care, for admission to the Agency, and obtains physician orders and approval of the plan.
  • Provides skilled nursing care to patients in their home in accordance with agency policy and procedures, physician orders, and accepted nursing practice.
  • Teaches patients, family members and other caregivers in the therapeutic regime in order to foster patient health and independence.
  • Implements the plan of care and coordinates the patient's care in cooperation with other health professionals.
  • Continually re-evaluates the patient's clinical status and reports changes in the patient's condition to the physician, case manager, supervisor, and other personnel involved in the patient's care. Performs follow-up on identified clinical issues.
  • Uses knowledge of community resources to advise patient and family needs of resources available pertinent to patient/family with an emphasis on continuing care needs after discharge.
  • Participates in patient centered conferences and contributes to the implementation of the plan as appropriate.
  • Orients and supervises care provided by home health aides in accordance with Agency policies.
  • Assumes responsibility for completion of all documentation according to Agency policies.
  • Participates in the orientation and ongoing training of RN staff and nursing students.
  • Assumes responsibility to maintain and upgrade professional knowledge and skills through attendance and participation in continuing education and in-service education programs.
  • Participates in the Agency's quality improvement program as required.
  • Supports the Corporate Compliance Program by adhering to the Twin Tier Home Health Policies, Procedures & Department Manuals of the organization. Employee will attend all mandatory in-services and annual training programs relating to Corporate Compliance. Assures that activities that employee is responsible for comply with any federal and state regulations. Reports and corrects any such conflicts which may arise.
  • Demonstrates knowledge of and adheres to Twin Tier Home Health policies and HIPAA regulations governing confidentiality and security of protected health information for both manual and electronic patient records, charts, information, and other confidential information including (but not limited to) employee records, computer access codes and passwords.

Position Qualifications

Minimum:

  • Graduate of an approved RN program OR a non-approved program supplemented by content equal to an approved program.
  • License and current registration to practice as a registered professional nurse in New York State.
  • Two years’ experience as a Registered Professional Nurse required OR equivalent of education and experience.

. Preferred:

  • Bachelor Degree in Nursing.
  • Med/Surg/Certified home care experience.
