Patient Care Pharmacy II, Ambulatory Care Pharmacy

United Health Services is seeking a Patient Care Pharmacy II to join our Ambulatory Care Pharmacy. Ambulatory Care Pharmacists will practice in an integrated outpatient setting that will involve:

  • Direct patient care and medication management for outpatients through collaborative practice agreements with providers within the practices
  • Disease State Management
  • Development of long term relationships with patients
  • Coordination of care
  • Patient education and self-management
  • Comprehensive medication reviews
  • Patient monitoring
  • Document patient encounters in the electronic medical record
  • Refer patient for additional care as necessary
  • Screen patients for a variety of clinical conditions
  • Recommend and administer immunization such as influenza and pneumococcal vaccines
  • Anticoagulation Management

This is a BENEFITS-ELIGIBLE/32 Hours Week position.

Shift: Rotating Days/ Nights/ Weekends
Hours per week: 32
Salary range: $53.02-79.52/hour Depending on experience

Patient Care Pharmacy II Overview

The Patient Care Pharmacy II Pharmacist ensures the correct medication is administered to all patients. This includes clinical appropriateness of the medication as well as the product delivery in an appropriate manner.

Patient Care Pharmacy II Key Responsibilities and Expectations

  • Interprets and, if necessary, clarifies orders / prescriptions written by physicians and other authorized practitioners and intervenes when clinically appropriate.
  • Ensures that orders / prescriptions are filled according to recognized professional standards, state and federal laws, and departmental policies.
  • Communicates with patients where necessary in all prescription counseling and educational activities required by law.
  • Oversees and supervises technical staff during distributive activities.
  • Prepares and supervises the preparation of products and / or parenteral admixtures.
  • Inspects and oversees inspection of medication storage areas.
  • Participates in any drug information programs in the department.
  • Maintains accurate controlled substance documentations as required by UHS Pharmacy department and all governing regulatory agencies.
  • Prepares and executes for signature all pharmacy related documents and reports to meet regulatory requirements.

Position Qualifications

Minimum:

  • A Bachelor’s of Science Degree (B.S.) or Pharm.D. in Pharmacy from an accredited college or university.
  • Licensed as a Pharmacist in New York State or NYS internship permit.

Preferred:

  • One year experience as a pharmacist.
  • Advanced Cardiovascular Life Support (ACLS) within one year of hire.
