United Health Services is seeking a Peer Support Specialist to join our Hep C Grant Team in Binghamton, NY. This is a BENEFITS-ELIGIBLE position.

Peer Support Specialist Overview

The Peer Support Specialist provides peer support, information, and advocacy to individuals at risk for or diagnosed with Hepatitis C or individuals at risk for acquiring HIV. Candidate will possess a strong understanding of Hepatitis C and substance use disorder or HIV prevention. Must be a current or past consumer of mental health, substance use, co-occurring behavioral health services, HIV, or HCV treatment program or PrEP treatment program.

Peer Support Specialist Key Responsibilities and Expectations

  • Outreach: Conduct regular outreach targeted at reaching people at risk for or living with Hep C, at risk for acquiring HIV, or people living with HIV. Outreach activities will include Hep C prevention education or PrEP education including accessing PrEP services, HIV education, drug treatment education, linkage to care services, and providing applicable referrals/referral follow up.
  • Patient Navigation: Work closely with patients diagnosed with Hepatitis C to access and complete treatment. Assess participants for immediate needs that pose a barrier to treatment such as lack of insurance, transportation issues, housing, food insecurity, etc. Work with patient and Hepatitis C Team to resolve these issues. Escort patients to appointments as needed. Assist Wellness Coordinator with medical appointment and lab appointment reminders. PrEP Peer will work closely with PrEP Specialist to assist patients on PrEP to access and complete treatment. Assess participants for immediate needs that pose a barrier to treatment such as lack of insurance, transportation issues, housing, food insecurity, etc. Work with patient and PrEP Team to resolve these issues. Escort patients to appointments as needed. Assist PrEP Specialist with medical appointments and lab appointment reminders. HIV Peer will work closely with RAP Specialist to assist with case management including assessing participants for immediate needs that pose a barrier to treatment such as lack of insurance, transportation issues, housing, food insecurity, etc. and help to resolve these issues. Escort patients to appointments as needed.
  • Data collection: Maintain accurate documentation of program activities. Document all patient progress in electronic medical record.
  • Education: Attend all required trainings and any additional training as needed to improve skills and knowledge surrounding HIV, Hepatitis C or PrEP and HIV prevention, substance use disorders, harm reduction, motivational interviewing, etc. Work towards becoming a NYS Certified Peer.

Position Qualifications

Minimum:

  • High School Diploma of Equivalent

License/Certification

  • Peer Support Specialist Certification or obtain certification within 12 months of hire.
