Nurse Navigator, Oncology, Wilson Square, Johnson City

United Health Services is seeking a Nurse Navigator to join our Medical Oncology Clinic in Johnson City, NY. The Nurse Navigator is a specialized health care professional who follows patients with a designated condition(s) or needs. The Nurse Navigator will focus on five areas of service: clinical critical thinking, condition education, community outreach, outcomes management and health and wellness promotion. This individual acts as a liaison for the patient and families addressing their concerns and questions about their disease(s) and/or condition(s). Additional responsibilities are to identify and qualify patients for specific programs as well as community education efforts, such as health screenings, promotions, and serve as an educator and liaison for the health care team. This is a BENEFITS-ELIGIBLE/Full Time/ Salary position.

Salary range: $30.58- $45.6/hour, depending on experience


Nurse Navigator Overview

UHS Medical Oncology is located at 30 Harrison St. #100 in Johnson City, NY 13790. The facility is open from 8:00 AM- 4:30 PM Monday through Friday.

Nurse Navigator Key Responsibilities and Expectations

  • Identify caseload based on presence of designated condition(s)
  • Facilitate transitions of care through the continuum both within and outside of the UHS network. Leads programmatic development efforts based upon feedback from patients, outcomes and competitive landscape
  • Recognize trajectory of disease process and tailor patient-centered interventions
  • Educates patient/family on condition status changes (ie, red flags)
  • Establish patient-centered goals and navigation care plan
  • Communicates identified patient condition status changes to patient/family and Care Team
  • Coordinate and provide patient/family education and care
  • Identify patient/family learning style and health literacy in order to provide patient/family education on the appropriate level.
  • Identify barriers to care and provide appropriate follow up including community resource referrals, revision of care plan goals, inclusion of additional Care Team members, etc
  • Elicit patient engagement in the health care system during and beyond navigation relationship
  • Empower patient/family in ongoing self-management of designated condition(s)
  • Promotes health and wellbeing by incorporating a holistic approach to condition management
  • Participation in UHS and community committees and forums regarding designated condition(s) and related outcomes
  • Advocate for patients/families during navigation relationship
  • Identify and respect patient choices and circumstances related to a patient’s lifestyle and literacy
  • Provides patient/family education related to Advance Directives
  • Collaboration with Palliative Care and/or Hospice
  • Appropriate, comprehensive documentation of navigation interventions in the electronic medical record
  • Triages and assesses patients for early identification of signs and symptoms to prevent complications and ensure timely intervention

Position Qualifications


  • Bachelor’s Degree in Nursing or proof of completion within 3 years from date of hire
  • NYS Registered Nurse license
  • Basic Life Support (BLS) certification (or obtained within 6 months of employment)
  • Valid NYS Driver’s license
  • 5 years health care experience


  • PA Registered Nurse license
  • Nurse Navigator Certification
  • Condition-specific experience in either/both inpatient or outpatient setting(s).
