  • Position Overview
    • The Nurse Case Manager provides direct patient care and oversees the nursing needs of assigned patients from admission to discharge. They are responsible for ensuring patients receive quality nursing care in a financially responsible manner and working with the Nursing Coordinator and interdisciplinary team to develop and implement a patient specific plan of care in accordance with the physician’s order.
  • Position Requirements
    • Education/Experience
      • Minimum Required:
        • Graduate of an approved RN program OR a non-approved program supplemented by content equal to an approved program.
        • Two years experience as a Registered Professional Nurse required OR equivalent of education and experience.
      • Preferred:
        • Bachelor degree in Nursing.
        • Med/Surg/Certified home care experience.

    • License/Certification
      • Minimum Required:
        • License and current registration to practice as a registered professional nurse in New York State.
      • Preferred:
        • Not Applicable.
