  • Position Overview
    • Provides personal care and expanded scope of task activities to patients, under the direction of the Licensed Home Care Services Agency (LHCSA) Registered Professional Nurses. Provides rehabilitative exercises under the supervision of the Twin Tier Rehabilitation Therapists. Reports to Home Health Aide Supervisor for guidance and assignment and to Twin Tier Registered Professional Nurses for supervision of patient care.
  • Position Requirements
    • Education/Experience
      • Minimum Required:
        • High School Diploma or Equivalent
      • Preferred:
        • Experience as a certified Home Health Aide desirable.

    • License/Certification
      • Minimum Required:
        • Completion of NYSDHD approved training program for certified Home Health Aide.
      • Preferred:
        • Drivers License
