Home Care Case Manager RN, Delaware County

United Health Services is seeking a Home Care Case Manager RN to join our Twin Tier Home Health Team in Binghamton, NY. This is an in home, traveling nurse position; each Case Manager is assigned 20-30 patients within Delaware County. The Home Care RN can control their schedule and have a fulltime career and manage family and home responsibilities at the same time. Salary range: $34.66-52.00 hour, depending on experience, plus mileage reimbursement.

Home Care Case Manager RN Overview

The Home Care Case Manager RN manages the nursing needs of assigned patients from admission to discharge ensuring patients receive quality nursing care provided in a financially responsible manner. Case Managers are responsible for the provision of skilled nursing services and teaching in the home. Works with the Nursing Coordinator and the interdisciplinary team to develop and implement a patient specific plan of care in accordance with the physician’s orders. RN’s will be expected to travel (average expected travel 25-50 miles/day)

Key Responsibilities and Expectations

  • Makes the initial assessment visit in the patient’s home and determines whether the patient is appropriate for admission to the agency. Develops a patient specific plan of care that makes the best use of agency resources. Maintains knowledge of OASIS guidelines, with the support of the Quality Manager, to ensure the patient’s care needs are accurately reflected in their case mix.
  • Provides skilled nursing care to patients in their home in accordance with agency policy and procedures, physician orders, and accepted nursing practice. Performs ongoing assessments of patient care needs and amends the patient plan of care as indicated.
  • Teaches patients and their support systems the knowledge and skills necessary to foster independent management of their disease process (including co-morbidities) and to reduce the risk of re-hospitalization.

  • Collaborates with other agency disciplines and community resources to foster successful attainment of goals and to support a positive transition from home care to independent management of care needs.
  • Initiates and participates in patient centered case conferences to ensure the patient receives quality care that fosters safety and independence in the home. Seeks support of the care manager and/or supervisor as necessary to meet patient and agency goals.
  • Assumes responsibility for the completion of all documentation according to agency policies, including submission of completed documentation within 24 hours of visit. Assists with the maintenance of the clinical record to demonstrate progress toward established outcomes and in accordance with quality and regulatory standards.
  • Orients and supervises care provided by home health aides in accordance with agency policies.
  • Participates in the orientation and ongoing training of new employees and nursing students.
  • Assumes responsibility to maintain and upgrade professional knowledge and skills through attendance and participation in continuing education and in-service programs.
  • Supports the Corporate Compliance Program by adhering to the Twin Tier Home Health Policies, Procedures and Department Manuals of the organization. Employee will attend all mandatory in-services and annual training programs relating to corporate compliance. Assures that activities that employee is responsible for comply with any federal and state regulations. Reports and corrects any such conflicts which may arise.
  • Demonstrates knowledge of and adheres to Twin Tier Home Health policies and HIPAA regulations governing confidentiality and security of protected health information for both manual and electronic patient records, charts, information, and other confidential information including (but not limited to) employee records, computer access codes, and passwords.
  • Participates in agency’s quality improvement program as requested.

Position Qualifications

Minimum:

  • Graduate of an approved RN program OR a non-approved program supplemented by content equal to an approved program.
  • License and current registration to practice as a registered professional nurse in New York State

Preferred:

  • Bachelor Degree in Nursing.
  • Med/Surg/Certified home care experience.
