United Health Services is seeking an Electronic Education and Care Plan Coordinator to join our Nursing Education team in Johnson City, NY. This is a benefits-eligible position.

Shift: Days, 8:00 a.m. - 4:30 p.m.
Hours per week: 32
Salary range: $30.72 - $46.08/hour, depending on experience

Electronic Education and Care Plan Coordinator Position Overview

The Electronic Education and Care Plan Coordinator promotes the interdisciplinary, evidence-based patient care process at UHS Hospitals which supports the best possible quality of care in a cost-effective and efficient manner. The position provides resources that support the bedside clinician in meeting regulatory standards, consistent with all policies and procedures.

UHS Wilson Medical Center is a 280-bed hospital and Level II Trauma Center. In this position, you’ll serve a medical facility that is a regional leader in cardiac surgery, neurosurgery and neurosciences, cancer care, maternity and high-risk obstetrics, neonatal intensive care, blood disorder treatment, and physical therapy and rehabilitation. Wilson is a bustling hospital and is a 24-hour designated stroke and chest pain center.

Electronic Education and Care Plan Coordinator Responsibilities and Expectations

  • Facilitates the development of interdisciplinary plans of care and patient teaching tools based upon evidence-based practice that guides the delivery of care and promotes optimal patient outcomes.
  • Coordinates the implementation and use of online nursing reference tools for evidence-based practice, including Mosby’s Nurse Consult, Mosby’s Nursing Skills, and Zynx.
  • Facilitates the review/revision of all plans of care per regulatory requirements and in collaboration with Patient Care Services Policy & Procedure Committee and Nursing Practice Council.
  • Plans, coordinates, implements, monitors, and evaluates plan of care projects as appropriate. Coordinates the activities of the project teams during implementation. Communicates with nursing management and staff to obtain ideas, concerns, information, and advice in all phases of the project implementation and on-going post implementation. Evaluates the functioning of the project and takes corrective action as necessary.
  • Participates or leads in nursing education as it relates to nursing practice resources including education for automation, care planning, and online resources.
  • Collaborates with Clinical Information Systems to maximize care planning as an integral component in nursing documentation using online systems to create and maintain care planning functions.
  • Identifies methods to monitor compliance and associated clinical outcomes with nursing care planning.
  • Participates as a member of the Nursing Practice Council and Policy and Procedure Committee.
  • Provides education opportunities for staff nurses and others involved in care planning as indicated.

Position Qualifications

Minimum:

  • Bachelor’s Degree in Nursing
  • Three (3) years experience in an acute care setting

Preferred:

  • Master’s Degree in Nursing
  • Greater than three (3) years experience in Nursing
  • Experience as an educator or trainer
  • Technology skills including word processing, spread sheets, presentations and UHS informatics systems (email, Intranet, Internet)
  • License/Certification
    • Minimum Required:
      • Current New York State Registered Nurse License
