Clinical Dietcian Overview

UHS Home Care is dedicated to meeting the nutritional and educational needs of our patients and community individuals by providing food services. The Clinical Dietician provides medical nutrition therapy for disease management to produce patient positive outcomes in the outpatient setting.

This is a Home Care Position and the Dietician will be providing services in patients homes.

Key Responsibilities and Expectations

  • Makes home visits and/or reviews diet intake studies to screen patients for their level of nutritional risk and prioritize patient care.
  • Completes proper office procedures and paperwork
  • Assesses the patients’ nutritional status and develops an interdisciplinary nutritional care plan.
  • Responds to all physician/nursing consults with appropriate recommendations documented in the medical record.
  • Collaborates with other health care team members about patients’ nutritional care through informal dialogue and interdisciplinary patient care conferences and documents these in the patient record.
  • Provides nutritional counseling to patients/caregivers based on evaluation of nutritional needs.
  • Conducts medical record audits, as assigned.
  • Gathers quality improvement data, as requested.
  • Conducts nutrition presentations to clinical staff, other health care professionals and community, as needed.
  • Acts as a resource in the area of nutritional expertise for the agency.
  • Assists with determining competencies of peers.

Position Qualifications

Minimum:

  • Bachelor’s Degree in Nutrition or related field plus successful completion a supervised practice program for nutrition.
  • NYS certification as a dietitian/nutritionist within 1 yr of hire date.
  • License and current registration to practice in NYS.
  • CPR Certification
