Clinic Coordinator RN, Wilson Square Cardiology, Johnson City, New York

United Health Services is seeking a Clinic Coordinator to join our Cardiology Team in Wilson Square, Johnson City, NY. This important Coordinator position is vital to maintain the highest level of teamwork this practice is known for. This position is a Benefits Eligible Salaried position.

Clinic Coordinator RN position Overview

The Cardiothoracic Clinic Coordinator, RN manages patient care from pre-assessment testing to hospital discharge and post-operative care. This position navigates multiple roles and responsibilities to ensure smooth clinic function, coordination of care, coordination of resources and optimum patient experience. The incumbent coordinates testing and treatment for patients, collaborates with physicians and other health care workers to provide comprehensive care, monitors the patient care process to ensure quality outcomes, and collects data to evaluate and improve care. Also serves as a patient advocate, maintains adherence to UHS’ Service Delivery Standards, takes responsibility for other department and organizational objectives and works closely with the Medical Director of the Program. The RN is responsible and accountable for compliance to all standards of practice and professional development in accordance with the New York State Nurse Practice Act, United Health Services Hospitals and Nursing Division policies, ANA Scope and Standards of Practice, Code of Ethics for Nurses, and the Bill of Patient Rights.

Clinic Coordinator Key Responsibilities and Expectations

  • Acts as a Key Point- of contact for patients and their caregivers.
  • Coordinates pre- and post-operative appointments and testing for patients
  • Coordinates resources required for procedures
  • Works closely with surgery scheduler to plan patient procedures
  • Expedites logistics for each patient by addressing travel needs, streamlining admission, and processing referrals
  • Facilitates communication among Heart Team members
  • Ensures patients are seen by appropriate providers and their cases are presented to Heart Team for final determination of care plan
  • Maintains TAVR Workflow data and builds patient slide-sets using Microsoft Office tools
  • Supports the maintenance of TVT Registry data for each patient.
  • Educates patient and family in heart health promotion and disease management using available resources.
  • Formulates and evaluates educational strategies for patients, families, and the community
  • Collaborates and leads patients, families, and all members of the Heart Team in coordinating patient care.
  • Collaborates and leads patients, families, and all members of the Heart Team in coordinating patient care.
  • Participates in and facilitates Performance Improvement activities, monitors PI data and assists with the development of Performance Improvement plans.
  • Collaborates with Heart Team members to promote and facilitate the compliance and understanding of Joint Commission, NYS DOH, Infection Control and all regulatory standards.

Position Qualifications

Minimum:

  • Bachelor of Science Degree
  • Current State NY RN license.
  • Documentation of current CPR certification required within 1 week of start of employment.
  • At least 2 years’ experience in a health care setting with evidence of having obtained proficiency in those areas emphasized in the Registered Nurse curriculum.

Preferred:

  • 1 year experience with patient Care Coordination
  • 1 year experience with Microsoft Office
